Job Description

9:00 A.M - 5:00 P.M

SUMMARY DUTIES AND RESPONSIBILITIES
  • Review the medical records of patients in order to identify those whom will require SUD intervention and counseling.
  • Interview patients to assess their insight into their addiction and readiness to accept treatment.
  • Initiates patient referrals to appropriate in-house or community-based SUD programs.
  • Completes Social Work Assessment for these high/moderate-risk patients as needed.
  • Maintains effective communication and provides supportive counseling, and crisis intervention to the patient with regards to addiction treatment options and services.
  • Collaborates as a liaison with Chemical Dependency department staff/programs.
  • Documents all Social Work interventions in the patient's electronic medical record in accordance with departmental and hospital policies.
  • Maintains effective communication with the interdisciplinary team and participates actively of interdisciplinary discharge planning rounds and records plans/recommendations in the patient's medical record.
  • Provides shadowing for new ED Leads staff and coverage to CATCH when designated. Acts as a resource for SW staff.
  • Assist in the orientation and training of new staff of the SW department and other disciplines as needed.
  • Will provide individual and group therapy to patients receiving services through the Chemical dependency department as needed.
  • Complies with requirements for supervision, training, Performance improvement projects, Quality Assurance and Research Studies.
  • Understands and follows the escalation process as needed.


Minimum Qualifications

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
