Job Description
1. Mentors and assists less experienced social work staff in social work techniques and serves as a
resource in various aspects of advanced social work practice.
2. May supervise graduate and undergraduate social work students, as well as volunteers and other
auxiliary personnel, as needed.
3. May assist in the provision of clinical services only under supervision of a New York State LCSW,
licensed psychologist, or psychiatrist or perform such functions independently as a LCSW

Minimum Qualifications

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.

Department Preferences

Preform psychosocial assessments of incoming patients on medical units
Preforms a Specific needs assessment of services required to develop and implement care plans
Provides individual and group services as indicated by the needs of the patient.
Facilitates appropriate referral services for patients
Advocates for patients with community agencies
Provides case management services with initial assessment and referrals and regular reassessment for services.
Attends and Represent the hospital at the community planning and coordinating group
Attends and participates in Clinic Team rounding on assigned team/unit
Contacts community agencies to facilitate referrals and develop linkage agreements.
Advocate and negotiate with related social and community agencies to ensure services for patients.
Compiles and submits daily/monthly departmental statistics
Attends Social Work Staff meetings and special training programs as they occur
Completes and maintains confidential files
Complete required documentation in a timely fashion, according to assigned area requirements and in a professional manner
