Harlem Hospital Center has been a bedrock of the Harlem community since 1887. Duties & Responsibilities

Under Supervision provides Social Work Services to patients and their families in the Ambulatory Care Services Clinics. With Moderate latitude for the exercise of independent judgement, in addition to performing typical described below.

SUMMARY OF DUTIES AND RESPONSIBILITIES:
  • Promotes a positive and pleasant attitude conducive to the workplace which enhances the Mission, Vision and Value Statement of NYC Health + Hospitals and Harlem Hospital. Meets all compliance standards/requirements such as Annual Physical Evaluation and PeopleSoft ELM Trainings and enhances personal and professional development through external educational offerings.
  • Completes the assessments of service needs and documents progress notes in Epic as per the Department policies. Completes referrals to social welfare and/or necessary service agencies and treatment/service resources and conducts and documents all appropriate and necessary follow-up activities to ensure service activation and patient receipt of services. Provides supportive /crisis intervention counseling to patients, families as appropriate. Advocates for patients and their families with various community agencies to ensure services are in place.
  • Consults and collaborates with interdisciplinary team and social work colleagues in all relevant services to plan and coordinate treatment plans, services, referrals and follow-up activities and meet the needs of the patients. Interviews Domestic Violence victims to assess the needs for concrete services and/or counseling services. Provides counseling and or makes referrals as required. Reports suspected cases of child abuse o neglect to the New York State Registry. Interviews patients /relatives/family members, facility staff and community members regarding patient's ability to function in the community.
  • Participates and attends social work meetings, grand rounds, social work conferences, clinic weekly meetings and team rounds, as part of the multidisciplinary team. Participates in designated hospital and departmental meetings, initiatives and programs requiring social work service provision. Supervises and evaluates the work performance of social work staff supervised and social work students when assigned. Participates in clinical /administrative supervision with the department Associate Director. Participates in Performance Improvement/PI Projects/Special Projects/Committees as well as Hospital Wide Initiatives when assigned.
  • Performs other Social Work Functions as assigned by the Social Work Administration.
  • Obtains consent to communicate patient information, with patient consent, to and from family members, significant others, friends and collateral professionals in the community who are involved in patient care in compliance with HIPPA regulations /standards.
  • Promotes a positive and pleasant attitude conducive to the workplace which enhances the Mission, Vision and Value Statement. Follows all relevant policies as it relates to her job function as a Level II Social worker of NYC Health + Hospitals and Harlem Hospital.


Minimum Qualifications

For Assignment to Level II
  1. Valid license as an LMSW issued by the NYSED; or
  2. Valid license as an LCSW issued by the NYSED.
