Summary

The System Director reports to the Chief Nursing Officer (CNO), charged with designing and leading the long-term vision, strategy, and implementation of the Lee Health Hospital at Home (H@H) operations in a dyad partnership with the Medical Director, Hospital at Home. In addition, the System Director will collaborate with the broader organizational executive team in the establishment of the H@H Division. The H@H division will establish home-based care and virtual services to provide clinical care in the patients home that would traditionally be provided to patients in the inpatient hospital setting. The System Director, Hospital at Home will be responsible for areas including, but not limited to, clinical operations, H@H financial performance, clinical practice learning and training, oversight for all day-to-day nursing, and emergency preparedness. The System Director must demonstrate a deep understanding of hospital based care and the ability translate that knowledge and experience to establish hospital-level care in a home setting. In home care is inclusive of clinical, operational, regulatory, and financial leadership. The System Director will lead the launch, evolution, and continuous optimization of the Lee Health H@H care delivery operations.

Primary Duties and Responsibilities:
  • Design, lead, and facilitate the creation of systematic patient identification, capacity management, and patient flow models. Operationalize models to spread and scale based on growth projections.
  • Build a stable and scalable clinical operations function that can shift and adapt in a rapidly changing and growing environment.
  • Establish leading practices for H@H nursing functions, including in-person home visits, virtual visits with H@H patients and their caregivers, and virtual command center nursing oversight.
  • Partner with organizational leaders to develop strategies to engage with governmental and commercial payers in fee-for-service and value-based arrangements required to economically scale Lee Health Hospital at Home.
  • Change management agent, leading organizational efforts to build support for H@H as a viable and preferred option for a growing census of appropriate patients.
  • Lead all emergency preparedness for H@H clinical operations in coordination with the broader Lee Health Emergency Preparedness Team.
  • Build the learning and development strategy and for H@H clinical team members. Ensures appropriate clinical education is available to support the onboarding, development, and continued competency growth of clinical employees.
  • Manage the clinical workforce and daily clinical operations. Oversee the development, implementation, and monitoring of clinical standards for all clinical employees.
  • Analyze clinical workflows and procedures to identify and implement continuous improvement opportunities.
  • In collaboration with others in leadership, develop operational plans for clinical support areas.
  • Develop, plan, and implement policies, procedures, and protocols to support clinical and other patient care services.
  • Develop and support internal controls to ensure that clinical operations policies and operating procedures are followed, and necessary controls are effective and efficient.
  • Serve as an essential thought partner to the CNO, Medical Director Hospital at Home, and overall organizational leadership associated with the launch and growth of the H@H Division.
  • Serve as a leading voice across Lee Health and represent the health system externally, advocating best practices for the H@H Division.
  • Manage the operating model for H@H care in coordination with the Medical Director, H@H.
  • Ensure that the voice of the patient and their caregiver is prioritized throughout the division.
  • Lead build out of H@H including: workforce, digital, procurement, financial, regulatory, and legal programs.
  • Oversee the H@H virtual command center to effectively deploy resources through the system.
  • Negotiate contracts and partnerships with vendors supporting clinical operations. Develop and manage the annual operating budget and capital budget.

Requirements

Education:Masters degree in nursing required, or a Bachelor of Science in Nursing combined with a Masters degree in Business Administration, Healthcare Administration, or related field.

Experience: Experience in a Nursing Director or equivalent role in the acute care setting. Hospital at Home experience preferred. Nursing or APP background required, 10+ years in healthcare leadership, experience with cross-functional teams and projects, experience building or leading highly matrixed teams, knowledge and experience managing contracts.

License:Registered Nurse Licensed in the State of Florida or Licensure requirement met within 60 days of hire for non-Florida residents.
