Description:
RESPONSIBLE FOR:
Responsible for providing clinical nutrition services, including nutrition assessment, modified diet formulation, selfmanagement
training and nutrition intervention to patients/residents. Ensures patient satisfaction, quality care,
regulatory agency compliance, and good public relations are achieved through the safe and efficient use of
resources.

Qualifications:

Minimum Education Requirements:

Bachelors Degree

Minimum Licensure:

Licensed Dietitian in Georgia

Additional Qualifications:

Registered Dietitian Nutritionist (Commission on Dietetic Registration)

Certified Clinical Transplant Dietitian (CCTD) or Certified Nutrition Support Clinician (CNSC)

Minimum Experience Required:

2-5 years of acute hospital experience

Key Responsibilities:

Provides individualized medical nutrition therapy to ambulatory and hospitalized transplant patients.

Apply medical nutrition therapy, standards of care.

Provide professional guidance and education in nutritional management of the transplant outpatient.

Nutritional evaluation surgical candidates utilization CMS selection criteria for transplant consideration.

Participation in inpatient rounds with MD team.

Participation in program-specific quality assurance and process improvement initiatives.

Contribute and participate in weekly selection committee meetings with the MD team.

Completion of Medicare time study reports.

Actively pursues continuing education.

KNOWLEDGE, SKILLS & ABILITY:

Ability to evaluate medical record and patients status to determine, develop and implement pre and post-surgical nutritional plan of care.

Ability to critically analyze patients condition to optimally manage and improve patient outcomes.

Ability to effectively present information and respond to questions from medical staff, healthcare professionals, managers, clients, customers, and the general public.

Ability manage a workload. Have excellent time management, verbal and written communication skills.
