JOB TITLE: WOUND CARE NURSE-VERSION 3

MAIN FUNCTION:

Under the direction of the Director of the Center for Wound Healing, the Wound Care Nurse has the responsibility and accountability for utilizing the nursing process in the prevention and treatment for acute and chronic wounds throughout the Arnot Health. Functions primarily as a direct patient care provider of wound care and also provides wound care education to that population.

DUTIES AND RESPONSIBILITIES:

1. Identifies, assesses and manages patients with potential/actual alternation in skin integrity.

2. Evaluates patient response to treatment and progress of wound healing, making adjustments and modifications as indicated.

3. Assists the physician with wound care procedures.

4. Provides for patient and family educational needs that may include: general skin care, wound care, nutrition, circulation, and diabetic education.

5. Collects patient health data.

6. Analyzes the assessment data in determining diagnoses.

7. Identifies expected outcomes individualized to the patient.

8. Develops a plan of care that prescribes interventions to attain expected Outcomes.

9. Implements the interventions identified in the plan of care.

10. Evaluates the patient's progress toward attainment of outcomes.

11. Systematically evaluates the quality and effectiveness of nursing practice.

12. Evaluates one's own nursing practice in relation to professional practice standards and relevant statues and regulations.

13. Acquires and maintains current knowledge and competency in nursing practice.

14. Interacts with, and contributes to the professional development of, peers and other health care providers as colleagues.

15. Decisions and actions on behalf of patients are determined in an ethical manner.

16. Collaborates with the patient, family, and other health care providers in providing patient care.

17. Uses research findings in practice.

18. Considers factors related to safety, effectiveness, and cost in planning and delivering patient care.

19. Accepts accountability for individual professional practice.

20. Respects and recognizes each patient as a unique individual and provides holistic nursing care.

21. Involved in planning and delivery of care across continuum of care.

22. Communicates effectively and efficiently between shifts and across disciplines.

23. Identifies appropriate care plan for each patient and delivers plan of care consistently.

24. Demonstrates leadership and availability to nursing staff.

25. Identifies own learning needs and develops education plan accordingly, including continued development of technical and critical thinking skills.

26. Participates in unit, Division of Nursing, and Arnot Health councils/committees, as appropriate.

27. Foster quality improvement in nursing practice.

28. Provides age appropriate care to all patients.

29. Is responsible for attending all annual mandatory educational programs as required by position.

30. Employee understands and demonstrates the importance of satisfying the needs of the customer/patient by interacting with him/her in a friendly and caring way, being attentive to the customer's needs, both psychologically and physically, and by taking the initiative to maintain communication with the customer in order to provide a secure and pleasant experience with Arnot Health.

EDUCATION & EXPERIENCE:

Current NYS RN nursing licensure. Excellent communication skills and writing ability. Previous wound care experience preferred.
