Summary

Lee Health is seeking an experienced RN for a new Ambulatory RN Care Coordinator position. Come join our new Advanced Lung Care Center and be a part of our efforts inthe early detection and treatment of patients with lung cancer!

The Advanced Lung Care Center will be located at the Regional Cancer Center and we will eventually expand to Bonita Community Health Centers. This program includes the lung cancer screening and lung nodule programs. The RN will play an integral role in helping coordinate care for those under lung cancer surveillance and those with early-stage lung cancer. This position requires strong technology skills as we utilize Epic, Nuance Follow Up Manager, and Excel to track patients in our programs. This position has limited hands-on patient care and requires more phone calls to patients to navigate them through the care continuum and coordinate care according to treatment plan.

The Ambulatory RN Care Coordinator utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, promote and maintain optimal health status of assigned patients. This position functions in a collaborative partnership with the patient, physician and the care team in the office setting. This position serves in an advocacy role to ensure the delivery of quality, efficient, and cost-effective patient-centric health care services. The Ambulatory RN Care Coordinators case load consists of moderate to high risk patients.

Additional Requirements
