Job Description

Position Overview: The Non-Clinical Admissions Liaison will be responsible for managing admission referrals in primary referring hospitals and ensuring a smooth discharge process for patients transitioning from a hospital setting to our PAC facilities.

Responsibilities:
• Establishes the patient's eligibility for Post-Acute care in collaboration with Central Admission department.
• Interacts with a variety of hospitals social service/case management staff regarding patients discharge plans
• Assess referral to establish suitability of prospective admissions.
• Communicate/ meet with patients and families to provide information of H+H Post-Acute facilities.
• Coordinates with healthcare facility staff to ensure that each patient has a smooth transition through the discharge process.
• Verifies health insurance coverage for patients with central admission assistance and support.
• Coordinates communication between patients and their physicians, case managers and other medical staff.
• Develops relationships in the community on behalf of the Post-Acute care facilities.
• Responds electronically to Allscripts referrals in timely manner.
• Obtain new high acuity inquiries and assesses special patients to evaluate care needs prior to admission, and directs to appropriate center
• Evaluate inquiries, makes the decision in conjunction with central admission and communicates information to case manager, admissions coordinator, and facilitates patient's smooth transition into center
• Obtain accurate information from physicians, patients, and referral sources regarding the expected discharge plan, and communicates this information to central admission and facility as needed.
• Develop relationships with hospital social workers, physicians, hospital discharge planners and administrators, clergy, attorneys, home health agencies, families, etc. to understand the need for program services and to communicate services offered which meet these needs
• Demonstrate growth in quality hospitals referrals and admissions, as reflected in an increase in inquiries and admissions from assigned hospitals to HCR centers
• Implementing marketing plans to achieve sales goals in collaboration with central admission and PAC operational leadership.
• Cultivating relationships with sources to generate repeat referrals.
• Participating in facility's marketing education, ensuring knowledge of their resources and maintaining knowledge of competitor facilities.
• Providing facility tours and conducting presentations for potential referral sources and patient families.
• Collaborating with Central Admission personnel regarding referrals, evaluations and marketing activities.
• Communicating activities and concerns to supervisor.

Completing required paperwork, managing correspondence and maintaining data as required

Minimum Qualifications
1. Master's degree from an accredited college or university; or 2. Baccalaureate degree from an accredited college or university; and one (1) year of administrative and operational support or related experience; or 3. A satisfactory equivalent combination of training, education, and/or experience, in which each year of relevant training and/or experience can be substituted for 30 semester credits from an accredited college or university. However, all applicants must at least have a High School Diploma or its educational equivalent.

Department Preferences
• Bachelor's Degree preferred
• Must demonstrate exceptional verbal and written communication skills.
• Must be customer service oriented with strong interpersonal skills
• Experience working in the healthcare industry, preferably long term care setting.
• Knowledge of Post-Acute care and knowledge of Medicare and Medicaid eligibilities
• Must have excellent computer skills and knowledge of databases.
• Must have knowledge of electronic health records, medical terminology and coding.
• Must have excellent organizational skills and be an effective time manager and multi-tasker.
