Job Description

General Purpose of Job:

Responsible for implementing assigned aspects of the Bereavement Program following organization policies, procedures, and practices. Provides grief counseling to individuals, families, children, and groups to facilitate healthy grief processes. Facilitates appropriate referrals for longer-term treatment, as needed.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
  • Assist in the creation, implementation, and completion of care plans for bereaved as assigned.
  • Implement community education and support programs.
  • Follow-up counseling in homes, office, or by telephonic or virtual means for families as requested and/or identified by staff as high risk for complicated grief.
  • Facilitate support groups, workshops, and education groups. Assist in development of new group offerings.
  • Participate in Interdisciplinary Team meetings for Hospice House and Home Hospice Teams as assigned.
  • Assist in planning and implementation of Memorial Services.
  • Provide Hospice staff education and support regarding accumulated grief.
  • Assist in developing and evaluating bereavement programs responsive to community bereavement needs.
  • Update bereavement resources in community and staff libraries.


Job Requirements

General Requirements

• Licensed by State of Michigan LARA as LLMSW or LMSW • BLS certification required; can be completed within orientation period upon hire

Work Experience

• One year minimum experience of providing grief & loss support &/or education • One year minimum hospice experience

Education

• Requires a minimum of a Masters Degree in Social Work (MSW), Counselling, Family Studies, or related

Specialized Knowledge and Skills

• Ability to operate a vehicle and have a valid driver's license • Use of up-to-date telephone and computer technology • Excellent assessment, communication and documentation skills with ability to creatively problem solve. • Able to manage diverse patient caseload, potentially stressful situations, and interpersonal conflict. • Able to develop and maintain a working relationship with community agencies in area and network effectively. • Able to utilize time effectively and efficiently, to organize workday and set priorities. • Able to work independently and within a team environment.
