The Care Management Support Professional 2 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Professional 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

The Care Management Support Professional 2 Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures. Decisions are typically related to schedule, daily operations plan. Performs escalated or more complex work of a similar nature and supervises a group of typically support and technical associates; coordinates and provides day-to-day oversight on operational plans to associates. Ensures consistency in execution across team. Holds team members accountable for following established policies.

Required Qualifications
  • Associate's degree
  • Minimum 4 years of supervisor experience
  • 3+ years Healthcare, DME, Home Health, Infusion, or medical industry experience
  • Proficient Microsoft Office technical experience with Word, Excel, Outlook
  • Decision making skills regarding own work assignments and priorities
  • Knowledge and experience working with Medicare/Medicaid & commercial/private insurance benefits, medical authorizations/billing
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences


Preferred Qualifications
  • Bilingual (Spanish and/or Creole)
  • Healthcare supervisory experience
  • Associate degree in healthcare field

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
