Job Summary:
The Virtual Care Coordinator helps to facilitate aspects of virtual care services and assists in the development and implementation of new virtual care services that indirectly impact care of the patient. The coordinator helps to bridge the gap between clinical practice and technical application to drive best practice for virtual care service use. The Virtual Care Coordinator demonstrates knowledge skill and competency when applying evidence-based practices. The coordinator will be responsible for developing relationships with multi-disciplinary providers and the local staff at facilities that receive virtual care services to assist in education, creating increased access to care. The coordinator serves as an advocate for both the end-users at receiving facilities as well as the clinical team delivering the virtual services. The coordinator will serve as a liaison between the virtual care users and the Virtual Care Director to identify opportunities for improvement in different virtual care services.
Education:
Required:
Associate degree and/or equivalent work experience
Preferred:
BSN/bachelor's degree
Experience:
Required:
Greater than 3 years of patient experience required. Evidence of leadership abilities required.
Knowledge of general health care principles and practice skills; knowledge of organizational functions, policies and regulations; knowledge of current trends and new developments; knowledge of principles and practices of nursing, applicable to those areas requiring special education.
Preferred:
Experience working within EHR or other technical platform

Department Position Summary:
Collaborates with the Director and service line Administrators to address specific clinical and operational education needs for the staff using virtual care services. The Virtual Care Coordinator is responsible for ensuring coordinated quality, seamless access to care. The coordinator manages and facilitates all training and education on the specific virtual care initiatives delivered through Erlanger. The coordinator offers guidance and supports the end-users of the designated virtual care platform used to deliver the virtual services. The coordinator demonstrates knowledge skill and competency when applying evidenced based practices. The coordinator will be responsible for developing relationships with multi-disciplinary providers and the local staff at facilities that receive virtual care services to assist in education, creating increased access to care. The coordinator serves as an advocate for both the end-users at receiving facilities as well as the clinical team delivering the virtual services. The coordinator will serve as a liaison between the virtual care users and the Virtual Care Director to identify opportunities for improvement in different virtual care services.

The coordinator is responsible for educating/training the facility end-users and providers on the virtual care platform and helps to keep any new implementations on track.
He/She will assist, as deemed appropriate, in research activities. He/She will work with the leadership team to collect data, track outcomes and support the strategic planning processes. He/She will develop and deliver quarterly reports on identified outcome metrics attained through performance improvement activities. He/She will work to ensure compliance standards.
