Job Description

Purpose of Position:

The Clerical Associate Level Ill is responsible to ensure the availability of patients' medical records; responding to medical record requests from various agencies, including subpoenas and court orders. Ensures all requests are tracked and processed in a timely manner and compliant manner. Collaborates with and works cooperatively with representatives of Verisma and other vendors to coordinate the release of medical records. Perform chart analysis and chart completion duties. Safeguard and maintain Protected Health Information in a safe, secure environment, where records are guaranteed total security and confidentiality in accordance with established regulatory standards, NYC Health + Hospitals, and departmental policies, as well as state and federal laws.

Duties and Responsibilities:

General tasks and responsibilities will include:
• Receive, sort, and distribute all incoming mail to appropriate personnel, departments, and/or vendors
• Provide clerical support to HIM Director, HIM administration team, DRG Validator, Release of Information (ROI) and scanning team
• Monitor and complete requests or tasks via HIM work queues/work lists
• Respond promptly to telephone, walk-ins, and STAT retrieval for ROI requests
• Review authorizations for ROI in compliance with hospital policies and procedures, as well as state and federal laws
• Release medical information to patients, physicians, healthcare providers, reviewers, auditors, regulatory agencies, and various agencies including subpoenas and court orders, while ensuring strict adherence to Security and Confidentiality guidelines of the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
• Log and scan medical record requests including RAC, IPRO, DOH, payers, DRG Denials correspondence, and other medical record hard copies (ex. Fetal Monitor Strips) into Epic for tracking and timely processing/reporting
• Generate electronic or hard copies of medical records for release by mail, fax via secure file transfer, drop zone, portals, and all regulatory compliant means to fulfill medical record requests
• Perform chart analysis duties and communicate with providers to ensure chart deficiencies are addressed timely
• Assist with chart retrieval, chart tracking to receive/create/send/modify/update, scanning, quality assurance, chart completion, demographic changes, and other administrative duties, as requested by the Director or designee
• Receive calls and relay information to department leadership and members of the team
• Meet all compliance standards/requirements such as annual physical evaluation/assessment, Annual In-Service Training, Fire Drill Participation, etc.
• Supervise and instruct a small unit or group engaged in clerical activities
• Participate in Performance Improvement activities and HIM committees
• Adhere to the Mission, Vision, and Value Statement of the organization in all endeavors
• Exhibit flexibility on work assignment, workplace, site designation, and Tour of Duty
• Performs other work and activities, as assigned

Minimum Qualifications
1. A high school diploma or its educational equivalent; and
2. Six (6) months of full-time satisfactory clerical experience and/or experience interacting with the public, such as processing sales; taking, processing, and delivering orders; obtaining and dispensing information; listening to and resolving customers concerns, problems, and complaints; and/or performing related customer service activities.
