Job Description

PURPOSE OF POSITION

The Operations Support Coordinator (OSC)/Clerical Associate Level IV - is responsible for organizing the delivery of health care services that are provided by clinicians to patients referred to home care services. The OSC serves as a scheduler and customer service representative for our clinical staff and patients.

Duties & Responsibilities

AREAS OF RESPONSIBILITIES
The Home Care Scheduler is responsible to coordinate and maintain scheduling on all disciplines for Community Care patients and staff.
SUMMARY OF DUTIES AND RESPONSIBILITIES
1. Review of all incoming referrals in queue for acuity and projected Start of Care (SOC) by physician.
2. Arrange, create and organize initial appointments in Epic for patients in line with healthcare plan.
3. Arrange, create and organize all revisits in Epic for patients based on visit sets entered by clinicians.
4. Telephonically communicates with new patients/family to confirm demographics in Epic and availability for initial visit.
5. Update patient demographics as needed.
6. Update clinician templates in Epic as needed to reflect availability for scheduling.
7. Responsible to create and maintain all clinical staff schedules.
8. Monitoring and processing of scheduling work queues (Nursing, Rehabilitation, Social Work, Registered Dietician) and scheduling depots for upcoming appointments.
9. Provide notification or update of schedule changes to all services (clinical staff) involved.
10. Contact clinicians and inform them regarding day-to-day changes in schedule.
11. Effectively communicates with various departments to disseminate and receive information as requested or needed.
12. Responsible for the review of missed visits in HHA Exchange and communicating to physician when Home Health Aide (HHA) visit is not completed by entering missed visit in Remote Client.
13. With collaboration of Assistant Director of Nursing (ADN)/Director provides staffing for clinicians on sick leave, vacation or long-term leave.
14. With collaboration of ADN/Director enters assigned caregivers in Epic.
15. Monitor and follow up on messages made by patients/family with our answering system Messages Plus to facilitate optimal patient care.
16. Assist ADN/Director with reports in Epic (Patients Needing Recertification/SOCs Still in Scheduled Status).
17. Assist ADN/Director with Not Taken Under Care (NTUC) of cases in EPIC when needed.
18. Responsible for assisting to front-line staff including ADN, Directors, and clinical staff as needed.
19. Participates in Epic training when required.
20. Participates in bimonthly to monthly department meetings when required.
21. Performs other duties as assigned.

Minimum Qualifications

Qualification Requirements:
1. A high school diploma or its educational equivalent; and
2. Six (6) months of full-time satisfactory clerical experience and/or experience interacting with the public, such as processing sales; taking, processing, and delivering orders; obtaining and dispensing information; listening to and resolving customers concerns, problems, and complaints; and/or performing related customer service activities.

Skills Requirement:

Keyboard familiarity with the ability to type at a minimum of 100 keystrokes (20 words) per minute.

YEARS OF EXPERIENCE:
Eighteen (18) months of satisfactory clerical experience
