Responsibilities
Liaison for the area, assisting staff, coordinating and preparing communication. In addition they provide organizational support.
ESSENTIAL KEY JOB RESPONSIBILITIES
1. Initiate patient scheduling, including appointments in ancillary departments.
2. Track and schedule followup procedures and appointments.
3. Prepare letters, reports and correspondence in a correct, businesslike manner.
4. Initiate and follow through on precertification requirements, referrals and communication with patient's insurance company/PCP.
5. Sort mail on a daily basis, initiating action when appropriate (i.e., lab results, patient request for information, physician correspondence).
6. Organize and maintain patient charts, filing, obtaining correspondence and test results, and sorting results which need physician review prior to filing.
7. Obtains and communicates clinical information from patients, families, medical record and other members of the health care team to case managers.
8. Obtains information from third party payers to determine provider networks and discusses transfer options with the assigned social worker and/or case manager.
9. Completes all necessary paperwork and system for patient transfers.
10. Demonstrates accountability for patient safety and reporting concerns in an accurate and timely manner to members of the team.
11. Participates with collaborative practice teams.

Qualifications
Required Education and Experience:
- High School Diploma/GED
- Two (2) years of relevant experience
- Preferred - Strongly: Bilingual speaking Spanish
Required Minimum Knowledge, Skills, and Abilities:
- Computer skills required.
- Experience in a health care setting required, may include social service agencies, state, local or community programs/agencies.
- Requires excellent communication skills with members of the health care team, community facilities and vendors, and most important, patients at high risk for readmission.
- Demonstrates ability at selfdirection, patient advocacy, follow through, and evaluation of services rendered.
