The Discharge Team Lead will focus on enhancing Follow Up after Hospitalization (FUH) and achieving our Healthcare Effectiveness Data and Information Set (HEDIS) goals. This role involves overseeing a team that assists facilities in securing and documenting aftercare appointments for members who have been psychiatrically hospitalized and providing resources to support discharge planning.

This position is full time 40 hours per week Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7am - 6pm CST. It may be necessary, given the business need, to work occasional overtime.

This will be on the job training and the hours during training will be 7am to 6pm CST, Monday - Friday.

Primary responsibilities:

  • Data Analysis: Monitor team and facility performance using data and utilize to identify gaps, and address them through workflow modifications, resource reallocation, and escalation to oversight and provider networks as needed.
  • Collaboration: Work closely with case management, utilization management care advocates, network and other areas to ensure success
  • Process Improvement: Identify possible additional actions that that could improve FUH, develop workflows, and implement with the team.
  • Support UM Oversight role in data research, root cause analysis, and mitigation through solution development and problem solving as needed

Required Qualifications:

  • High School diploma, GED or equivalent work experience
  • Must be 18 years or older
  • Ability to work 40 hours per week from 7am to 6pm CST.
  • 3+ years experience leading a team
  • 2+ years experience in managed care environment
  • 5+ years experience in healthcare
  • FUH/HEDIS knowledge
  • Previous experience analyzing data
  • Experience presenting data to others clearly and concisely
  • Adept at learning and teaching systems and troubleshooting system issues
  • Advanced knowledge of Microsoft Office - Microsoft Word (ability to create and edit correspondence in documents), Microsoft Excel (ability to create, edit, sort / filter, modify PivotTables, leverage advanced analytical formulas: match / if / lookup, and apply nested formulas in spreadsheets), Microsoft PowerPoint (ability to create and edit slides in presentations), and Microsoft Outlook (email and calendar management)

Preferred Qualifications:

  • Provider/Facility outreach experience
  • Experience working in a matrixed environment
  • Previous experience working in behavioral

  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
