Duties & Responsibilities

A.General Responsibilities:
1. Facilitate the development and implementation of treatment guidelines, protocols, formulary changes as approved by appropriate committees.

2 Coordinate the selection and safe use of alternative medications in response to medication shortages.
3. Support the pharmacy's department clinical endeavors, Review the use of targeted medications for compliance with current pharmacoeconomic program.
4. Provide clinical consultation and clarification to practitioners.

5 . Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
6. Monitor drug therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug laboratory test interactions adverse drug reactions, appropriateness of drug and dose, and therapeutic duplications.
7. Provide accurate, adequate and timely drug information to the professional staff.

8 . Contribute to the publishing in the pharmacy newsletter.

9, Supervise and conduct continued education to the pharmacists and pharmacy students.

1 0. Prepare and review drug use evaluation criteria, antibiotic sensitivity reports, adverse drug reaction reports, and pharmacy interventions summary to various committees.

1 1 . Participate in hospital educational programs (e.g. Coumadin, Diabetes, Congestive heart failure clinics.).

12 Coordinate policies and procedures for clinical activities (e.g. therapeutic interchange, unacceptable abbreviation, look and sound-alike drugs, medication events reporting, etc.).

1 3 . Participate in the quality improvement activities of the department.

1 4. Collect analyze data and maintain documentation of all clinical activities.

1 5 . Establish productive, collaborative relationship with staff department members and other discipline.

1 6 . Sustain professional competence and promote development through participation in in-services, workshops, seminars, and professional organizations,

1 7 . Precept and mentor pharmacy students for both St John's University and A.M. Schwartz Colleges of Pharmacy.

1 8. Train pharmacists and pharmacy students to participate in drug use evaluation.

1 9 . Facilitate discharge/transfer planning and smooth transition related to medication orders.
20. Complete special projects as assigned by supervisors, assistant director or director of pharmacy.

B. Infectious Diseases:

1 , Participate in daily patient care rounds with the ID and medical teams.

2 Promote pharmacist involvement in antimicrobial selection, dosing, monitoring for selected agent.
3. Monitor conversion of intravenous to oral antibiotics.

4, Serve as a resource to staff for the evaluation on the compliance of antibiotic usage criteria.

5 Participate in ID rounds provide recommendations and educate to the ID team and house staff.
6. Develop policies or practices that encourage selection of the antibiotic regimen with the narrowest spectrum of activity based upon culture and sensitivity result.s.
7. Coordinate education to pharmacists on appropriate antimicrobial selection.
8. Continuously review drugs of choice and monitor utilization.

9, Make formulary presentations regarding new antimicrobial agents,

1 0. Develop programs that target high cost and/or specialized antimicrobials,

C. Critical Care:

1 . Participate in daily patient care rounds on the intensive care units and medical team (or supervise qualified individuals).

2 Discuss medication order clarifications with the prescriber and document any changes as intervention.
3. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form and drug interactions.
4. Provide pharmacokinetic consultations as required.
5. Prevent, detect, monitor, document and report adverse drug reactions and medication errors.

6, Promote the use of the formulary by converting non-formulary orders to formulary, when possible, but coordinate procurement of non-formulary drugs when necessary.
7. Develop and conduct target drug programs and drug usage evaluations on a consistent basis.
8. Document clinical interventions and cost avoidance as part of the departmental improvement initiatives
9. Monitor the use of expensive medications to assure its use consistent with approved criteria (e.g. IVIG, albumin, etc.)
10. Facilitate the personnel development in critical care for pharmacists.
11. Provide presentations, publications, and other informative activities on critical care and other drug-related topics to pharmacists and other healthcare professionals.
12. Participate actively in critical care team meetings, which include discharge management.

Minimum Qualifications

Maintain current New York State pharmacy license and registration.

Hospital experienced preferred, but not required.

BS or PharmD Pharmacy degree
