Job Description

Pharmacist prepares and dispenses medication orders per physician request according established policies, procedure, and protocols.
1. Review and accepts orders on the hospital computerized physician order entry (COE) for accuracy prior to dispensing.
2. Fill unit-dose medication carts by each nursing unit.
3. Compound and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition preparations accurately.
4. Issues controlled substances to patient care areas and maintains records as required by law.
5. Communicate pending issues to other pharmacy personnel prior to leaving each shift (hand-off).

Replace medications removed from emergency carts accurately within established time frames.
7. Ensure integrity of emergency cart locks.
8. Replace expired medications in emergency carts.
9. Ensure that emergency cart expiration date is within time period specified by departmental policy.
10. Prepare requests for medication from the stock room.
11. Store medications and hazardous chemicals safely and securely
12. Review STAT orders within 1-hour penod
13. Review non-Stat order within 2-hour period.

Minimum Qualifications

Qualifications:
• Maintain current New York State pharmacy license and registration " Hospital experienced preferred, but not required
• BS or PharmD Pharmacy degree
