Job Description

AREA OF RESPONSIBILITY
(List summary of duties and responsibilities):

A. Unit Dose Responsibilities:
Pharmacist prepares and dispenses medication orders per physician request according established policies, procedure, and protocols.
1. Review and accepts orders on the hospital computerized physician order entry (COE) for accuracy prior to dispensing.
2. Fill unit-dose medication carts by each nursing unit.
3. Compound and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition preparations accurately.
4. Issues controlled substances to patient care areas and maintains records as required by law.
Communicate pending issues to other pharmacy personnel prior to leaving each shift (hand-off).
Replace medications removed from emergency carts accurately within established time frames.
1. Ensure integrity of emergency cart locks.
2. Replace expired medications in emergency carts.
3. Ensure that emergency cart expiration date is within time period specified by departmental policy.
4. Prepare requests for medication from the stock room.
5. Store medications and hazardous chemicals safely and securely
6. Review STAT orders within 1-hour period.
7. Review non-Stat order within 2-hour period.

B. Non-Unit Dose Responsibilities:
Ensure safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols. Know the policies and procedures of the department and hospital standards.
1. Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, appropriateness of drug and dose, and therapeutic duplications.
2. Contact prescribers when needed and document interventions.
3. Detect and reports suspected adverse drug reactions and medication errors in a timely manner.
4. Sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection,
5. Provide clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
6 . Provide drug information to medical staff, nursing, other healthcare professionals, patients and their families.
Document all clinical activities and interventions accurately and completely.
Participate in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; and maintain logs, records, and other documentation as assigned.
Attend pharmacy staff meetings.
1. Complete all competence/skills assessment requirements
2. Perform other duties as assigned by supervisor.
3. Abide to National Patient Safety Goals (NPSGs).
4. Convert IV to PO regimen for patient safety.
5. Arrive to work area on a timely basis.

C. Investigational:
1. Meet with investigators, study monitors, and other in coordinating study logistics.
2. Receive copies of investigational and study procedure and protocols. Obtain IRB, HHC approved letters, informed consent form, and inventory control form. Review documents and determine pharmaceutical materials and professional services required to satisfy the study descriptions.
3. Provide randomization, blinding, and other activities as required.
4. Develop procedures for preparation, dispensing and distribution of study drugs.
5. Assist in establishing and maintaining adequate and easily retrievable record keeping for protocols, study subjects, drug inventory, manufacturing, and dispensing investigational drugs.
6. Prepare drug data sheet and documents for nursing and pharmacy personnel.
7. Maintain standards which assure the quality, proper storage, and safe use of all investigational agents and related supplies.
8. Ensure required documentation is completed and retained, and that study drugs are handled according to the requirements of sponsoring agencies.
Responsible for and assist with the development of statistical reports and activity summaries.

1. Supervise and direct pharmacy support personnel.
2. Verify the daily activities of pharmacy technicians.
3. Participate in the performance appraisal of pharmacy support personnel.
4. Work independently as per professional standards.
5. Organize and prioritize work assignments.
6. Ensure pharmacy services are provided in a timely manner.
7. Identify and replace outdated and unusable drugs.
8. Answer the telephone, identifying self and department. Direct calls to appropriate personnel. Answers requests at the window,
9. Keep pharmacy areas and equipment clean, neat, and well-organized.
10. Work a minimum of 1 weekend, 1 night, 1 evening per month as required by the department.
11. Rotate to all areas as needed by the department.
12. Participate in multidisciplinary rounds.
13. Participate in patient care counseling activities.
14. Knows how to reconstitute, prepare, store and administer Covid-19 Vaccines

Qualifications:
Maintain current New York State pharmacy license and registration
Hospital experienced preferred, but not required.
• BS or PhamD Pharmacy degree

Minimum Qualifications

Possession of a valid NY State license to practice pharmacy.
