Day City Hospice is searching for a Full Time Manager, Continuous Care. The Continuous Care Manager is responsible for the supervision of the appropriate staffing of continuous care (CC) patients utilizing available staff and resources; the reconciling of payroll to ensure accurate and timely payroll and billing; and compliance with policies, procedures, and standards utilized in the care of continuous care patient and families.

POSITION REQUIREMENTS:

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience.
• Previous experience in hospice/home care setting with two years management or supervisory experience preferred.
• Currently licensed as an LPN/LVN in the State.
• Prior use of Electronic Medical Records (EMR), preferred.
• Maintains a current CPR Certification.
• Valid driver's license and proof of insurance is required
