Summary:
Responsible for the use of advanced nursing processes to identify high risk, acutely ill and patients with chronic disease. The role of this position is to improve health outcomes through a population health based platform. The position is accountable for assessing, planning, educating, monitoring, evaluating, and coordinating services for patient populations and their families, to foster quality, continuity and appropriate utilization of health care resources throughout the continuum of care. Facilitates the coordinated utilization of resources for maximization of health outcomes, patient/family satisfaction and financial outcomes. Assists patients, families, and caregivers in securing necessary medications, equipment, community resources to facilitate health and overall wellness. As a member of a multidisciplinary team, consults with other health care team members to coordinate the services of preventive care and chronic disease management.

Formal Education Required:
a. Current license to practice registered nursing in the State of Ohio.
b. BSN preferred.

Experience & Training Required:
a. Three (3) years clinical experience in care coordination.
b. Strong clinical experience with diabetic populations and other chronic diseases.
c. Strong clinical knowledge in promoting healthy lifestyles and preventive screenings.
d. One (1) year of Care Management preferred.
e. Knowledge of Epic electronic medical records software preferred.

Other Skills, Competencies and Qualifications:
a. Demonstrates the ability to manage time and promote departmental and professional growth.
b. Ability to use electronic software applications related to care management activities. (i.e. HealthyPlanet, payer portals to retrieve population health data, CMS portal data retrieval).
c. Act as a subject matter expert for clinical staff education materials and other resources.
d. Implementation of care management activities in their assigned office sites.
e. Completes and/or attends all required educational offerings.
f. Participates in continual improvement of care management processes.
g. Knowledge of current practices in population health.
h. Knowledge of quality improvement/performance improvement tools.
i. Proficiency with computer software programs, e.g. MS Outlook, Word, Excel, and PowerPoint.
j. Ability to set goals independently and prioritize work to balance competing deadlines in a professional manner.
k. Excellent customer service skills.
l. Excellent written and verbal communication skills.
m. Ability to manage multiple tasks.

a. Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently

$30.60/hr - $45.90/hr

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers. Please keep in mind that the range that is listed is the full base salary range. Hiring at the maximum of the range would not be typical.
