MDS Coordinator Opportunity at Victoria Crossing Rehabilitation Center

Are you passionate about elevating the quality of care for long-term care residents? Victoria Crossing Rehabilitation Center is seeking a dedicated MDS Coordinator to lead our care planning team. In this pivotal role, you will be at the heart of ensuring our residents receive the highest standard of care, working closely with our skilled nursing staff, multidisciplinary teams, and families.

Salary Range: $36 - $42 per hour

Key Responsibilities:

• Evaluate and Enhance Care: Oversee the quality of care for our long-term residents, ensuring their needs are met with excellence.
• Comprehensive Assessments: Conduct initial interviews and thorough assessments for new residents.
• Care Planning: Develop detailed care plans focused on both short-term and long-term goals, tailored to each resident's needs.
• Collaborative Leadership: Facilitate meetings with multidisciplinary teams, caregivers, and families to align on care objectives.
• Community Collaboration: Partner with community specialists to achieve the best outcomes for residents.
• Team Integration: Participate in Nursing management meetings, ensuring effective communication and collaboration at all levels.
• Education and Training: Lead in-service education sessions on MDS, RAPS, and Triggers, promoting knowledge and best practices.
• Compliance: Adhere to the MDS/Care Planning schedule, particularly for non-Medicare residents.

Qualifications:

• Florida RN License: Must hold a valid RN license in the state of Florida.
• Experience: At least 2 years of experience in a Skilled Nursing Facility.
• Expertise: Strong knowledge of MDS, Medicare, and the care-planning process.
