Position: MDS Coordinator
Location: Palm Beach Nursing
Salary: $36 - $42 per hour (based on experience)

Are you an experienced RN with a passion for enhancing the lives of long-term care residents? Palm Beach Nursing is seeking a dedicated MDS Coordinator to take charge of the MDS process and collaborate with our care planning team to ensure top-quality resident care.

Key Responsibilities:

• Assess and monitor the quality of care for our long-term care residents
• Conduct initial assessments and interviews for new residents
• Create individualized care plans with both short- and long-term goals
• Lead meetings with interdisciplinary teams, caregivers, and families
• Collaborate with community specialists to achieve care plan objectives
• Participate in departmental meetings as part of the Nursing management team
• Provide in-service education on MDS, RAPS, and Triggers
• Oversee the MDS/Care Planning schedule for non-Medicare residents

Qualifications:

• Valid RN license in Florida
• Minimum of 2 years of experience in a Skilled Nursing Facility
• Strong understanding of MDS, Medicare, and the care-planning process
