Job Description

Purpose of Position

The Nurse Educator is the clinical expert in evidence-based nursing practice in the areas assigned. Being a handson clinical expert in home care, He or She will work under the ANA Scope and Standards of Practice for Nursing Professional Development roles established by the American Nurse's Credentialing Center (ANCC) to include: Educator, Consultant, Researcher, Facilitator, Change Agent, and Leader. The Nurse Educator will also exemplify the eight nursing characteristics identified in the AACN Synergy Model (Clinical Judgment, Facilitation of Learning, Response to Diversity, Clinical Inquiry, Systems Thinking, Collaboration, Advocacy, and Caring Practices).
Working under the direction of the Chief Clinical Officer or Director of Nursing-Clinical Services, he/she will partner with newly hired skilled nurses for the Certified Home Health Agency and ensure that the competency-based orientation tool is completed during orientation by signing off assigned skills. Using the Nursing Professional Development (NPD) Practice Model, the Nurse Educator will apply the nursing process in the assessment of practice gaps, identification of learning needs, planning, implementing, and evaluation of progress and outcomes. In addition, the Nurse Educator will ensure that all regulatory documentations are completed within the regulated time frames.

Duties & Responsibilities

SUMMARY OF DUTIES AND RESPONSIBILITIES
• Review the self-assessment of new hires and tailor competency-based orientation to meet those needs. Provide validation of competency through the use of:
o Supervisory Field Visits
o Pre/Post-tests
o Return Demonstration
o Evidence of Daily Work
o Case Studies/Scenarios
o Exemplars
o Discussion/Reflection Groups
o Presentations
o Mock Events
o Transfer knowledge and skills specific to all the body systems
• Provides direct supervision to field staff, including on-site supervisory visits
• Evaluating personnel performance through observation, record review and home visits
• Track and Trend Progress of Nursing Competency and report findings to the Chief Clinical Officer or Director of Nursing-Clinical Services or Assistant Director of Nursing.
• Assumes the role of a Skilled Field Nurse if need arises

Implementing the Competency Based Orientation Tool

Disseminate the pathway to competency.
Provide Education and Review Competency on the Body Systems (Neurological, Pulmonary, Genitourinary, Cardiovascular, Gastrointestinal, Endocrine, integumentary, etc.).
Provide Education and Review Competency for Adult, Behavioral Health, and Maternal Child Health Nursing
Ensure all learning objectives are met.
If objectives are unmet, completes remediation plan and report plan to DON-Nurse Educator and A.D.N

Nursing Education and Training Design

Lead the Field Preceptor(s) and all educational directives for the home care program.
Strong Understanding of OASIS documentation
Strong Understanding of CHHA regulatory guidelines
Assist with the BLS Simulation Station under the direction of the Nurse Educator
Conduct skills training and supervisory field visits.
Helping staff set priorities and develop plans for care and services based on short- and long-term goals Participate in skilled nursing in-service, simulation exercises, and education programs utilizing skills lab Develop strategies for increasing awareness and competencies of clinicians in training programs Provide direct supervision of Field Staff, including on-site supervisory visits.
Assisting staff in teaching the patient information and skills necessary for quality care
Prepare and conduct remediation plan/ corrective action plan as necessary.
Assisting staff in teaching the patient information and skills necessary for quality care

Quality Improvement

Validate skilled nursing competence through the. evaluation of synthesis of skills, knowledge, and performance Participate in biweekly case conferences with ADNs and Nurse Educator to discuss progress of orientee Create learning workshops for incumbent staff who needs refresher trainings.
Maintain Active Participation on Shared Governance Councils and the System Preceptor Program
Provides inter-rater reliability assessments when needed to ensure accuracy in OASIS data collection.
Track, collect and maintain report of preceptor program analytics to support strategic planning, program evaluation and budgeting.
Knowledgeable in regulatory guidelines (QOH, CMS and Joint Commission) that dictate the delivery of home care services.
Continuously analyzes work processes and makes suggestions for improvement.

Assists with ongoing assessment and evaluation of clinical competency.
Assume the role of a field nurse to mitigate staffing challenges when needed.
Conducts evidence-based practice research as needed.
Represent the agency at various corporate-wide and industry events, as appropriate.
Participate with executive management in strategic planning, program evaluation and annual report preparation.

Minimum Qualifications

Education and Experience Requirements:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and
3. A master's degree in nursing, or a related nursing or education field, from an accredited college or university; and
4. Three (3) years of experience as a registered nurse in specialty area(s), including:
  • a. One (1) year providing nursing education and training in specialty area(s); or
  • b. Holds a valid and current certification as a Nurse Educator

5. Minimum 2 years of experience in Home Care or nursing education or demonstrated expertise in a clinically equivalent role to Onboard New Hires.
