MedStar Health is looking for a Program Coordinator at MedStar Good Samaritan Hospital. This Hybrid position is responsible for the daily management of Subacute at Home (\"SaH\"), a home-based subacute rehabilitation care program that is a collaboration of MedStar Good Samaritan's Center for Successful Aging and MedStar Health Home Care.

Primary Duties
  • Responsible for efficiently managing the daily operations and processes of the Subacute at Home ("SaH") Program. Coordinates home-based patient support functions for patients, patients' families, and caregivers.
  • Completes registration for new patients and updates information on existing patients in scheduling system(s) for both scheduling and billing purposes. Obtains necessary demographic and insurance information, referrals, pre-certifications, signatures, copies, etc. to ensure a smooth and timely process.
  • Follows up on outstanding matters, as needed. Directly communicates with patients and family members for scheduling, documentation, feedback, etc., as directed.
  • Provides coordination and support to SaH's interdisciplinary teams. Assists with a high volume of activities and rapid turnaround requests from Geriatricians, Nurses, Nurse Practitioners, Physician Assistants, Social Workers, Home Health Aides, Case Management, and other Allied Health providers.
  • Oversees clinical orders, completes documentation, provides billing support, submits prescription requests, manages orders for durable medical equipment, and other tasks, as needed.
  • Provides feedback and suggestions to improve workflow, best practices, and standard procedures.
  • Provides direction and leads weekly interdisciplinary clinical meetings (huddles) to track the patient's care and flow from admission through discharge of SaH. Maintains a schedule/calendar of activities and meetings, clinician locations, on-call schedules, and patient appointments while utilizing Electronic Health Record systems.


Qualifications
  • Bachelor's degree, Preferred.
  • Minimum of 2 years' experience working in a medical office, Required.
