Job Summary:

Are you a compassionate and skilled Registered Nurse looking to make a meaningful impact on patients' lives? Join our team as a Registered Nurse Care Manager (RNCM) and play a critical role in the care and well-being of our patients.

As an RNCM, you will be an integral part of our health care team, responsible and accountable to your patients. You will work closely with clinicians to develop and implement personalized care plans, ensuring that your patients receive comprehensive, coordinated care. Acting as an advocate, educator, and resource person, you will address the logistical, financial, emotional, educational, and social barriers that may impact care and compliance.

Key Responsibilities:
  • Care Management: Oversee the care management and coordination of a patient panel, ensuring that all aspects of the patient's needs are met through the health-team model of care.
  • Patient Advocacy: Serve as an advocate for patients, guiding them through their healthcare journey and providing education and resources to support their treatment plan.
  • Collaboration: Work closely with other RNCMs, nurses, and staff members to fulfill the needs of the team, often covering more than one center and team as needed.
  • Patient Engagement: Engage with patients regularly to assess and evaluate their clinical status, ensuring compliance with treatment plans and medication administration.
  • Care Rounds: Participate in care rounds and preplanning activities to ensure the highest level of care and coordination for your patients.
  • Communication: Maintain daily contact with clinicians, health center staff, patients, caregivers, and outside agencies/providers to ensure seamless communication and coordination of care.

Qualifications:
  • Active RN license
  • Experience in care management and patient advocacy
  • Strong interpersonal and communication skills
  • Ability to work collaboratively in a team-oriented environment
  • Flexibility to cover multiple centers and teams as needed
