Job Description

Under the direction of the System Chief Women's Health Officer, with some latitude for the exercise of independent judgement, is responsible for the planning, organizing, implementation and coordination of the full Maternal Home Psychosocial/Socio-Economic program in a New York H+H ambulatory OBGYN clinic.

The Maternal Care Social Worker plans, coordinates the activities necessary to support birthing persons across their pregnancy, childbirth and post-partum care journey. The primary goal of the Maternal Home social worker will be to improve the population health of obstetrical patients, by helping to reduce the incidence of severe maternal morbidity (SMM) and maternal mortality (MM).
  • Coordinates and conducts quality activities within the department regarding social work practice on complex issues.
  • Conducts psychosocial evaluations through interviews with patients and/or identified surrogates
  • Confers and consults with professionals in other disciplines to determine and implement a multi-discipline approach to patient care and to develop care plans, evaluate patient progress and make referrals based on the needs of the patient.
  • Provides outreach services, makes referrals to external partners and agencies, prepares patient to accept services, provides direct social work services on high-risk cases and may escort patients in connection with community activities and referrals as needed.
  • Participates in staff conferences and other related social work meetings, special studies, committees and quality improvements activities, programs and trainings as required.


Minimum Qualifications

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.

Department Preferences

  • Two years in a healthcare setting
  • Demonstrated ability to function productively and cooperatively with Clinical and Program Director, Medical and Administrative staff
  • High risk obstetric care for improving access to medical, psychosocial, mental and behavioral health and or emotional support, assessing socio-economic needs, providing nutrition and education guidance on topics such as breastfeeding and parenting support
  • Practice experience working with perinatal patients
  • Conducting psychosocial screening and assessment, and documentation in an electronic medical record system
