JOB SUMMARY:
Care Manager Team Leader will manage CHC members with chronic diseases, conditions or medically complicated needs through comprehensive evaluation and through coordination of resources. The Care Manager Team Leader will participate as a team member in CHC Care Management program. Care Managers will work collaboratively with utilization management, member services and the quality improvement departments.

- Education/Specialized Training/Licensure: RN current Texas License required; CCM required or must receive within 18 months of employment.
- Work Experience (Years and Area): Five years of clinical experience. One year case management experience preferred.
- Software Operated: Microsoft Office (Word, Excel, Outlook)
SPECIAL REQUIREMENTS:
- Communication Skills: Above Average Verbal (Heavy Public Contact)
- Writing /Composing (Correspondence/ Reports)
- Other Skills: Analytical, Medical Terms, MS Word
- Advanced Education: Advanced Training Specialty: CCM required or must receive within 18 months of employment
