Job Description
1. Acts as receptionist. Greets and direct patients, visitors and authorized personnel.
Refers non-authorized visitors to the Nurse-
• in - Charge.
2. Answer telephones including patient call bells. Takes and delivers messages. Locates doctors and nurses via telephone. Contact relatives and patients when so directed.
3. Prepares requisitions for repairs, supplies and maintenance. Takes periodic inventory of stationary supplies as directed.
5. Have additional addressograph pages readily available in the medicinal record for use. 1.e., Progress Notes and Doctors' Order Sheets.
6. Ensures that medical record contents are addressograph at all times.
7. completes demographic information on the following fonn:
• Institutional Transfers
n Transportation
• Home Care Referrals
n Hospital Rcfon-al Information
8. Upon transfer of patient, assemble all parts of the chart including Nursing Care Plan, MD Order, Respiratory Sheet, Intake and Output Records, Vital Sign Sheet, Diabetic Record. Medication Sheet, Neurological Assessment Sheets, excess charts (EKG Slips) and all reports (to be posted) to accompany the patient.
9. Upon discharge to the patient assemble all parts of the chart including items on the validation checklist. Deliver the chart to the Admitting office and sign the Admitting log.
10. At the beginning of each tour, compiles ward statistics. Counts patients for census. Enters admission, transfor or discharge data in Unit Activity Log.
11. Ensures that all patient s has an ID ann band.
12. Makes arrangements with ambulance service transport as directed.
13. Post notices on bu11etin board. Pick up doctor's Orders and complete and prepare labels and tags.
14. Completes 24-hour discharge notice submit to patient for signature notified change nurse if patient has clinical questions.
15. Prepares and maintains Kardex files, bedside cards.
16. Call 8888 Code as directed and monitor telephones during the 8888 Code.
17. Arranges for messenger service to transport specimen, mail and other items as needed.
18. Calls Medical Record Department for old charts requested and maintain a file for the old chart until patient is transferred or discharged.
19. Maintains stationary supplies, medical records, Kardex in a neat and orderly fashion and in accordance with established standards.
20. Maintains compliance with Infection control and Safety Guidelines as set forth in each manual t assist in providing a safe environment.

21 . Maintain a pleasant, well organized environment.
22. Assist in the orientation of new clerks.
23. Makes appointments for the discharge patients.
24. Monitors advance directive completeness, daily and submit data to ADON every Monday.

Minimum Qualifications
1. A high school diploma or its educational equivalent; and
2. Six (6) months of full-time satisfactory clerical experience and/or experience interacting with the public, such as processing sales; taking, processing, and delivering orders; obtaining and dispensing information; listening to and resolving customers concerns, problems, and complaints; and/or performing related customer service activities.
