Job Description

JOB FUNCTION

In this role, the successful candidate will work closely with internal and external requestors, our Release of Information (ROI) vendor and other requestors to ensure timely fulfillment of medical records requests. The Clerical Associate Level IV is responsible to manage ROI communications with patients, patient representatives, Risk Management, Corporate Compliance and Legal. Effective management of vendor deadlines, patient complaints, work queues and assigned staff. Responsible to safeguard PHI in a tamper-free environment and ensure protected health information is held and transmitted in accordance with the Privacy Rule of the Health Insurance Portability and Accountability Act of 1996, established Corporate, facility, and departmental policies as well as state and federal laws.

RESPONSIBILITIES AND DUTIES:

Responds promptly to telephone, walk ins and STAT retrieval/release of information.

Appropriately confirms receipt of record requests from patients and/or agencies

Conduct Data Integrity function in EMR - medical record amendments, demographic changes etc.

Assist with completion of patient forms, if required

Generate electronic or hard copies of medical records for release in person or by mail, fax, secure file transfer and other compliant means to fulfill medical record request.

Manage workload and follow escalation procedure to address staffing challenges.

Perform Master Patient Index inquiries in the Epic, QuadraMed, Billing and Chart Tracking systems to verify patient identity, medical record numbers and visit/episode dates.

Conduct Release of Information functions within Epic and QuadraMed EMR.

Monitor and complete request or task on work queues/worklists

Review HIPAA authorization forms and all documentation presented to ensure compliant records fulfillment in accordance with state and federal laws; returns all deficient requests to requestors for additional (satisfactory) information

Release of medical information to patients, physicians, healthcare providers, reviewers, auditors, regulatory agencies, to include subpoenas and Court Orders.

Participate in Performance Improvement Activities and HIM committees.

Train medical record personnel and other administrative staff.

Minimum Qualifications

A1. A high school diploma or its educational equivalent; and
2. Six (6) months of full-time satisfactory clerical experience and/or experience interacting with the public, such as processing sales; taking, processing, and delivering orders; obtaining and dispensing information; listening to and resolving customers concerns, problems, and complaints; and/or performing related customer service activities.
