Job Description

SCOPE OF POSITION
The Medication Discharge Tech will be responsible for logistically organizing, collaborating and/or delivering required discharge medications, self-care products, and post-acute care services to patients and/or families or caregivers. The Tech will ensure the successful hand-off to the appropriate post-acute services to enable the improvement of the patient's health management and experience. The goal will be for the Tech to involve the patient more actively in their own care. This will be done by efforts to ensure the patient is fully educated, engaged, and equipped to be successful in their self-care at home. This will be carried out during the time of intervention / discharge to make sure the patient's continuum of care has been covered and thoroughly reviewed with patient and /or families and caregivers.
The Tech shall interact with patient to ensure permission has been given; patients are eligible for services, have method of payment and are properly educated. The patient is both educated and provided as appropriate all post-acute services inclusive of medication (patient counseling on medication is provided by the pharmacist), products, and services. This individual is responsible for coordinating and tracking patient preferences, delivery abilities, and management of post-acute care service teams for the patient. The Tech shall be responsible for the delivery and procurement of all patient self-care needs upon discharge and & coordination of transition to home requirements. The Tech will communicate patient care feedback and requests that are regarding product/care needs to onsite management team and post-acute service team staff. The Tech is responsible for coordinating the clinical care team, case management and other staff associated with the discharge process in order to achieve the patients' self- care goals.

EDUCATION AND EXPERIENCE.
Two years of Pharmacy Technician or related Healthcare experience is also required. An Associate's Degree in healthcare, business or related field in conjunction with five or more years of Pharmacy Technician experience both in retail and/ or a hospital environment may be accepted in place of the National Certification. If the team member is not certified, they will need to obtain these credentials within six months of employment with Ballad. Familiarity with medical terminology is preferred. Team member must have advanced computer skills, be familiar with all Microsoft systems, and working with web based tools and/or point of sale/clinical systems.
Strong clinical skills, including an understanding of evidence-based care. Understanding of the health care system and its component parts including sites of care, delivery models, and the roles of various providers and health care professionals. Understanding of the community-based social service delivery system and its interaction with the health care system. Ability to manage patient complexity and multiple patients with diverse needs. Comfortable working autonomously and in a self-guided role. Experience in providing care in a hospital environment.
Valid and active Pharmacy Technician License in the appropriate state within 3 months of hire.
