In this role, you will provide direct support to the SDOH Program providing services to physicians and primary and/or specialty care patients. Major duties include, but are not limited to:

In this role, you will provide direct support to the SDOH Program providing services to physicians and primary and/or specialty care patients. Major duties include, but are not limited to:
- Surveying patients to assess their Social Drivers of Health (SDOH) needs as well as enroll them in the Social Care SDOH Program and assign them to the appropriate staff for support.
- Acting as liaison for the SDOH Community Health Workers (CHW) and Clinical Social Workers (CSW), patients and physicians in the practice,
- Assisting patients in accessing community services within and outside of the health system.
- Facilitating communication with community providers, physicians, and referring patients to appropriate resources and providing follow-up through telephone and EPIC.
