The New Mexico Department of Health is one of many executive agencies that strive to serve all New Mexicans as a societal and public health safety net provider, to which we have an emphasis on vulnerable and marginalized populations. The NE Region Health Promotion Program is within the Center for Access and Linkages to Health Care in the Public Health Division works with internal and external partners to support population health equity activities for communities throughout the 10 counties that make up the NE Region of New Mexico (Santa Fe, Los Alamos, Rio Arriba, San Miguel, Guadalupe, Taos, Mora, Union, Harding, and Colfax). These activities are intended to positively impact the health of communities within the Region. In other words, the Program develops, implements, evaluates, and generally supports health-related activities to support local needs of groups of people where they are currently and for generations to come and how the social determinant of health can be positively impacted to enhance how we as a community can thrive.

Key internal partners include other programs and bureaus within the Region and key external partners include tribal and county health councils, as well as not-for-profits and managed care organizations that support the work of councils and local communities.

Why does the job exist?

The Social & Community Service Coordinator - Advanced position is responsible for supporting the work of the NE Health Promotion Program by coordinating a range of activities for and with other Health Promotion staff. The position also works with other staff in the NE Region or in the Department of Health itself. Additionally, the position is responsible for coordinating activities with all active health councils potential organizations interested in becoming a health council, local communities, and agencies that support the needs of councils and communities such as not-for-profits and managed care organizations.

The position influences how activities are developed and implemented to support population health equity issues - and may also directly provide health education, build coalitions, facilitate groups, work on contracts or grants, or do other projects and tasks.

How does it get done?

The key responsibilities of the Social & Community Service Coordinator - Advanced position are to support the work of the NE Region Health Promotion Program by working with and for other Program staff on existing activities, proposing new activities to improve health equity issues identified by health councils and local communities, and engaging various internal and external partners to support existing and new activities. The position also supports other Program staff in assessing health councils' readiness for community health improvement planning, interpreting a variety of data, facilitating communications between a wide range of stakeholders on any given project or task, establishing collaborations and coalitions, proactively communicating program evaluation outcomes and other relevant activities on an Weekly basis with the program manager. The position will also support activities stemming from the Department and the Legislature, such as activities related to Public Health Essential Services 3, 4, & 5 and legislative directives among others.

The job of the Social & Community Service Coordinator - Advanced position is done to support the work of the NE Region Health Promotion Program, and ultimately done to support improving public health outcomes, and health equity for local communities through the Health Councils and other methods as needed in the NE Region.

Ideal Candidate

The ideal candidate for the Social & Community Service Coordinator - Advanced position is familiar with internal and external partners within the NE Region, and is a member of at least one local community, such as a health council. The ideal candidate should have knowledge and experience with:
* Writing community assessments and health improvement planning
* Working with tribal communities
* Facilitating groups and providing various trainings to groups
* Building community collaborations and establishing coalitions
* Teaching evidence-based practices or interventions, especially related to population health issues
* Comfortable and able to adapt to changing priorities, occasionally on short notice
*Proactive in communication with team and managers
*Responds to challenges with any aspect of a given task or job in a proactive and solutions-based approach.

Minimum Qualification

Bachelor's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care. Employment Requirements

Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification within six (6) months of date of hire as a condition of continued employment. Successful completion of required trainings of the NE Region Public Health Division and understanding the Health Insurance Portability and Accountability Act (HIPAA).

Working Conditions

Work is performed in an office setting which includes exposure to visual/video display terminals (VDTs), personal computers and phone usage is extensive. Work is also performed outdoors for outreach events, meeting spaces with other external organizations and all anticipated environmental elements associated. The position requires the ability to lift up to fifty (50) pounds; extensive travel may be required throughout the NE Region (ie. potentially within tribal communities of Eight Northern Pueblos, Jicarilla Apache, and counties of Santa Fe, Los Alamos, Rio Arriba, Taos, San Miguel, Guadalupe, Mora, Union, Harding, and Colfax).
