JOB SUMMARY:

Responsible for the daily supervision of operations of the Case Management and Social Work activities. Supports the integration of the department goals into the organizational culture. The Case Manager Supervisor is a registered professional social worker or registered nurse with education, knowledge, and experience related to all areas of Case Management. This supervisor participates in a team approach, composed of the physician, nurse, social worker, direct and indirect providers, community resources, and payers.

ESSENTIAL JOB FUNCTIONS:
  • Effectively creates and coordinates the overall transition and discharge plan of care based on initial

  • assessment in collaboration with social workers and the health care team, including patient, family, and

    external service organizations.
  • Educates unit staff and health care team regarding practice changes that affect care of specific complex cases,

  • pertinent case management documentation, and quality issues and acts as resource.
  • Attends/coordinates unit based multidisciplinary patient care rounds, discharge huddles, and/or readmission

  • huddles as assigned.
  • Demonstrates leadership in the provision of departmental services through a collaborative process designed

  • to integrate services/functions throughout DHL. Supervises daily operations and activities of the Case

    Management Department.
  • Operates departments of responsibility in such a way as to meet the fiscal goals of the organization.

  • Implements value added programs and services. Manages the satisfaction of department customers to include

    patients, employees and physicians through effective communication and appropriate responsiveness to

    customer issues.
  • Assists in managing productivity, salary cost, supply cost, and total cost for departments managed according

  • to budget expectations.
  • Assists the Director to develop standardize processes, enhance tools and assure the integration of daily

  • operations. Functions as a functional expert and liaison to staff to minimize fragmentation of daily

    operations.
  • Collects, analyzes statistical data from various resources and presents findings to meet external and internal

  • reporting requirement.
  • Maintains current knowledge of regulatory and compliance requirements, and policies and procedures for

  • utilization practices, processes and criteria.
  • Attends and participates in staff, committee, department, and other administrative meetings.
  • Identifies patients for weekly Patient Care Conferences, supervises/organizes conference, and assures appropriate documentation and follow-up are completed.
  • On-call rotation for discharge planning after 5 pm and on weekends.
  • This position serves patients of all populations and encompasses all other duties as assigned.Assists in developing annual departmental goals as applicable to patient population i.e., ALOS, admission and continued stay denials, cost/case, readmission rates etc.
  • Serves as Interim Director of Case Management when Director of Case Management is not available.

    Qualifications

    MINIMUM QUALIFICATIONS:

    Registered Nurse or Social Worker licensed in the state of Texas. Bachelor or Masters degree preferred. 5 years clinical experience in hospital setting preferred. 2 years supervisor experience required.
