Duties & Responsibilities

PURPOSE OF POSITION:

Serves as the primary clinician for the individuals with whom they work. In a sensitive and caring manner, they promote the notion of recovery and self-sufficiency, and lead the development and implementation of clinically sound, culturally relevant, and medically monitored treatment programs, case management, care coordination and other supportive and consultative services for patients in Substance Use Disorder (SUD) treatment programs. Participates in and/or performs functions related to counseling, diagnostics, treatment, treatment planning, rehabilitation, assessment, intervention and other supportive services for patients in SUD treatment. Ensures optimal utilization of resources and services and compliance with all applicable local, state, and federal rules and regulations and NYC Health + Hospitals standards of care to improve outcomes and the patient experience.

AREAS OF RESPONSIBILITIES:

Kings County Hospital Behavioral Health/SUD Treatment Programs Outpatient Programs (CDTOPS and OTP)

SUMMARY OF DUTIES AND RESPONSIBILITIES:
  • Conducts initial screenings ad assessments of patients, performs on-going toxicology screenings to obtain pertinent information about SUD and Co-Occurring Disorder(s).
  • Develops psycho-social histories; formulates appropriate diagnostic impressions; prepares accurate and timely screening, intake and assessment reports.
  • Makes decisions and maintains required records regarding appropriate Level of Care, at point of entry and on-going, using patient criteria tools (for example, LOCADTR) and clinical judgement, to ensure that a patient in need of SUD Treatment services is placed in the most clinically appropriate level of care.
  • Develops and continually monitors and updates, as indicated, treatment/recovery plans in collaboration with patients and members of the clinical treatment team, including establishment of short and long-term treatment goals, objectives, and interventions. Participates in the evaluation of treatment progress. Accurately and timely records treatment planning, consistent with established standards, applicable administrative guidelines and regulatory requirements.
  • Coordinates and provides case and care management services and activities; makes referrals and linkages.
  • Initiates and finalizes discharge and transition planning, in collaboration with patient and clinical treatment team. May make field visits and/or accompany patients to community services.
  • Maintains timely, accurate, and appropriate documentation in compliance with all applicable standards to ensure appropriate reimbursement.
  • Provides orientation to new patients, patients' families and significant others; discusses financial responsibility with patients and provides money management assistance.
  • Provides crisis intervention and support by responding to and following through in crisis situations; demonstrates good clinical judgement by seeking additional assistance and/or consultation in the management of crisis situations, when needed.
  • Conducts outreach activities, provides advocacy, and acts a liaison between programs, providers and community supports.
  • Participates in relevant staff meetings, case conferences and trainings; participates in special projects and internal/external quality assurance/performance improvement activities.
  • Participates in community outreach activities on and off campus.
  • Holds and maintains required certification(s) and completes required continuing education; submits appropriate documentation within established timeframe. Complies with NYC Health + Hospitals policies and procedures, and applicable to local, state and national requirements and guidelines.
  • Performs other related duties, as assigned.


Minimum Qualifications

Any individual working in a NYS Office of Addiction Services and Supports (OASAS)-Certified Treatment Program or newly-hired into the System in the title of Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment (MAT)" training.
For Assignment to Level I
1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Level 1 Certificate issued by the NYS Office of Addiction Services and Supports (OASAS); or
2. Is otherwise considered a Qualified Health Professional (QHP) based on licensure, certification and/or registration along with appropriate experience and/or training as defined in Section 800.4 (U) of the OASAS Treatment Services: General Provisions, Title 14 NYCRR Part 800 (https://oasas.ny.gov/system/files/documents/2022/09/800.pdf).
