Job Description

PURPOSE OF POSITION

Under direct supervision, the Assistant Addiction Counselor serves as an integral part of the multidisciplinary team, and assists in the screening, orientation, case management, care coordination and other supportive services and activities for patients in Substance Use Disorder (SUD) treatment, ensuring optimal utilization of resources and services and compliance with all applicable local, state, and federal rules and regulations and NYC Health + Hospitals standards of care to improve outcomes and the patient experience.

SUMMARY OF DUTIES AND RESPONSIBILITIES
• Initial screening of individuals seeking care, consent forms, providing basic orientation, discussion of financial responsibility
• Serves as liaison between treatment providers, departments, community organizations and/or other city agencies to help ensure access to appropriate services and resources throughout programs in the hospital and with-in the community
• Performs ongoing toxicology screening and prepares accurate screening reports
• Assists with the maintenance of electronic health records Performs outreach tasks
• Facilitates educational and skills training groups
• Provides basic crisis intervention and support; appropriately seeks additional assistance as needed
• Escorts patients into the community and makes home and/or site visits as needed
• Works collaboratively with members of the multidisciplinary treatment team, patient, family, significant others, collaterals, and other identified entities involved in the case in order to formulate and operationalize appropriate linkages and transition/aftercare plans
• Assists with liaison functions (identifying resources, completing applications, making referrals, collaborating with in-house team(s) and community services)
• Documents contacts and services provided in the health record in compliance with all applicable standards
• Participates in relevant staff meetings and discussions
• Assists with the preparation of care records and reports (including, but not limited to, OASAS reports) and maintains confidential patient records, in accordance with appropriate policies and procedures Participates in and/or completes quality assurance/performance improvement activities and special projects
• Demonstrates good verbal and written skills, organization, and the ability to work in an often complex and fast paced environment
• Demonstrates understanding and knowledge of principles and philosophy of prevention, person centered treatment, and recovery and how they relate to successful achievement of patient treatment goals and objectives through coordination and provision of case and care management services and activities that integrate supportive services, agency, professional and/or community resources in a planned framework of action, which addresses a patient's mental, physical, and/or behavioral needs Others tasks as directed or assigned

Minimum Qualifications

Any individual working in an OASAS-Certified Treatment Program or newly-hired into the title of Assistant Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment (MAT)" training and "Introductory Training in the Addictions" training.
1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Trainee Certification issued by the NYS Office of Addiction Services and Supports (OASAS); or
2. Bachelor's degree from an accredited college or university including or supplemented by twenty-four credits in Psychology, Sociology, Social Work or a related field; or
3. High School diploma or its educational equivalent and two (2) years of full-time paid satisfactory experience performing case management, care coordination, and related supportive functions in a substance use disorder treatment program; or
4. Satisfactory equivalent of education, training, and experience. However, all must have at least a High School diploma or its educational equivalent.
