Job Description

Purpose:

Under supervision, to provide responsible, quality clinical treatment to chemically-addicted patients.

Specific Duties and Responsibilities:
  • Counseling patients admitted to programs within the Chemical Dependency Division.
  • Accepted current knowledge-based skills in the treatment of substance use disorders.
  • Identifies specific problems of patients; is sensitive to cultural and spiritual needs of patients.
  • Listens to patient's complaints and promptly reports them to immediate supervisor.
  • Evaluates the progress/or lack of progress of patients.
  • Develops patient treatment plans and establishes short-term and long-term goals in consultation with clinical treatment team.
  • Participates in clinical team meetings in discussing individual cases and general patient progress.
  • Develops accurate psychosocial history of patients.
  • Conducts groups and psychoeducational training sessions with patients.
  • Maintains accurate, confidential, and legible medical records on each patient according to agency requirements.
  • Initiates and completes OASAS ADMISSION/DISCHARGE (PAS 46) forms.
  • Initiates discharge planning and referrals, acting as liaison for patient and completes Discharge Summaries.
  • Is knowledgeable of Hospital, Departmental and unit Policies and Procedures, and
  • Demonstrates the ability to perform his/her duties in accordance.
  • Conducts overdose prevention training and distributes naloxone rescue kits.
  • Is courteous and professional when interacting with colleagues, patients and the public
  • Other tasks as determined by department need and the Director of Chemical Dependency Services
  • Is knowledgeable of hospital and departmental policies and procedures, and demonstrates the ability to perform duties in accordance.


Minimum Qualifications

Any individual working in an OASAS-Certified Treatment Program or newly-hired into the title of Assistant
Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS
"Supporting Recovery with Medications for Addiction Treatment (MAT)" training and "Introductory Training in the
Addictions" training.
1. Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) - Trainee Certification issued by
the NYS Office of Alcoholism and Substance Abuse Services (OASAS); or
2. Bachelor's degree from an accredited college or university including or supplemented by twenty-four credits
in Psychology, Sociology, Social Work or a related field; or
3. High School diploma or its educational equivalent and two (2) years of full-time paid satisfactory experience
performing case management, care coordination, and related supportive functions in a substance use
disorder treatment program; or
4. Satisfactory equivalent of education, training, and experience. However, all must have at least a High School
diploma or its educational equivalent.
Note: Incumbent H+H Assistant Addiction Counselors deemed qualified under the prior qualification requirements
assigned to non-OASAS certified treatment programs as of July 1, 2018 may continue to function in their current
roles, but cannot be considered for assignment to OASAS-certified treatment programs without demonstrating the
qualification requirements indicated above, effective July 1, 2018.
