Job Description
Minimum Qualifications
1. A Baccalaureate Degree from an accredited college or university in Business Administration, Community Health, Psychology or related discipline with one year of full-time experience in a supervisory or administrative capacity in business management systems, general administration, health care and medical/patient systems administration; or,
2. A satisfactory combination of education, training and experience.

Job Description
- Providing outreach to patients to offer assistance scheduling appointments for needed services.
- Reviewing relevant information in the Electronic Medical Record
- Scheduling appointments for patients
- Tracking the results of outreach calls made to patients
- Understanding the value-based payment metrics driving patient outreach
- Communicating with patients in a caring, empathic, and culturally competent manner
Minimum Qualifications

- GED/High school degree preferred
- Some college education preferred
- Basic Microsoft Excel/Outlook skills
- Experience with PowerPoint is a plus
- Patient or customer outreach/interactions
- Customer service experience is a plus
- Healthcare experience is a plus
- Experience with Epic or EMR Systems is a plus
- Bilingual is a plus
- Strong communication skills
- Organization skills
- Experience meeting productivity targets is a plus
- Cultural competency and appreciation for health equity
- Experience with NYC H+H and/or the communities in which it serves
