Job Description

  • Providing outreach to patients to offer assistance scheduling appointments for needed services.
  • Reviewing relevant information in the Electronic Medical Record
  • Scheduling appointments for patients
  • Tracking the results of outreach calls made to patients
  • Understanding the value-based payment metrics driving patient outreach
  • Communicating with patients in a caring, empathic, and culturally competent manner


Minimum Qualifications
1. A Baccalaureate Degree from an accredited college or university in Business Administration, Community Health, Psychology or related discipline with one year of full-time experience in a supervisory or administrative capacity in business management systems, general administration, health care and medical/patient systems administration; or,
2. A satisfactory combination of education, training and experience.

Department Preferences

  • GED/High school degree preferred
  • Some college education preferred
  • Basic Microsoft Excel/Outlook skills
  • Experience with PowerPoint is a plus
  • Patient or customer outreach/interactions
  • Customer service experience is a plus
  • Healthcare experience is a plus
  • Experience with Epic or EMR Systems is a plus
  • Bilingual is a plus
  • Strong communication skills
  • Organization skills
  • Experience meeting productivity targets is a plus
  • Cultural competency and appreciation for health equity
  • Experience with NYC H+H and/or the communities in which it serves
