Job Description
NYC Health + Hospitals is seeking candidates with experience in the healthcare sector who have demonstrated strategic leadership skills, exceptional communication and organizational ability, and who have driven projects to successful completion. The Project Manager will work both independently and as a collaborative partner in a fast-paced environment.
NYC Health + Hospitals is creating a new Independent Practice Association (IPA) that will be responsible for establishing and operating a Medicaid Managed Care program for individuals experiencing homelessness.
NYC IPA aims to develop a whole-person, tailored approach to care that will advance health equity by transforming care delivery for populations at the highest risk for adverse health outcomes. This specialized IPA will bring together managed care plans, community-based organizations, social service providers, health systems, and behavioral health providers to promote accountability, efficiency, and excellence in care.
The Office of Behavioral Health (OBH) serves as the Central Office lead for behavioral health. OBH provides leadership, support and direction, including guidance on service delivery and policies for behavioral health; executive, clinical, liaising and advocacy with city agency partners, city, and state regulatory bodies. OBH also provides administrative leadership within NYC Health + Hospitals facilities to develop and implement managed care strategies and provides technical assistance, regarding clinical reengineering, quality management and technological enhancements.
Under the direction of the Senior Director of the Office Behavioral Health, the Project Manager is responsible for organizing, directing, tracking and coordinating the activities of the Behavioral Health care management services for patients with mental health and substance use diagnoses. This includes patients receiving care at all H+H BH inpatient, emergency room and outpatient services, special population IPA members, and individuals experiencing homelessness.
General tasks and responsibilities will include:
Minimum Qualifications
Educational and/or experience requirements will vary requiring diverse kinds and levels depending on the content, complexity and variety of project. Compensation levels will correspond to the requirements developed for the designated project.
In no event will the acceptable minimum be below:
1. A Baccalaureate Degree from an accredited college or university in a specialization discipline consistent with project content; and,
2. Four years of substitutable and relevant health care experience related to the objectives of the project; or,
3. A satisfactory equivalent combination of experience, training and education.

Job Description

- Assist to design, implement, and evaluate replicable and scalable care management strategies to achieve rapid, sustainable improvement in health outcomes and quality of life
- Assist in the collection and analysis of data and reports and dashboards and presentation material for the relevant projects
- Assist in the drafting of business plans for new initiatives related to this work
- Assist in the preparation of workflow and procedure documents
- Collaborate with administrative leadership, physicians and other clinical disciplines within Central Office and across the facilities for education, training/coaching and implementation
- Develop execution workplans and tracking tool
- Track progress based on project plans and coordinates activities and tasks to achieve milestones and outcomes according to plan
- Assists in preparing communication and communicate directly about different aspects of this work and assists in setting up meetings
- Liaise with various departments as needs such as HR, IT, finance, etc. to facilitate work related to the projects in scope (this includes Epic and recruitment/advertising)
