Job Description
The purpose of the Public Health Educator/Wellness Health Coach for the Ambulatory Care Service is to assist in the overall management of primary care patients diagnosed with obesity and subsequent chronic health conditions. The Individual will assist patients in making behavioral changes and promote tailored lifestyle recommendations regarding physical activity and diet. The lifestyle coach will collaborate with the clinical team and serve as a liaison to the chronic disease team, providers, patients and families for coordination of services.
He/she will offer group and individual sessions and should be comfortable utilizing a video platform for outreach.
Minimum Qualifications
1. A masters degree from an accredited college or university in public health education, education, public/community health administration, public administration or business administration, and one year of full-time satisfactory experience in developing public health education programs, including identifying target populations, conducting needs assessments, designing educational materials, planning educational presentations or workshops,
evaluating health education programs; presenting public health education programs; and/or counseling in areas such as communicable diseases, substance abuse, assault, sexual abuse, and/or family planning; or,
2. A baccalaureate degree from an accredited college and two years of full-time satisfactory experience as outlined in one (1) above.

Job Description

- Contacts and performs initial interviews with patients who are eligible for health coaching programs
- Assist patients in developing personalized care plans to establish behaviors that are health promoting (diet, exercise)
- Provides educational material in brochures, pamphlets, and books regarding wellness.
Serve as emotional support to patients and fosters behavioral change via motivational interviewing
Maintain a record of patient encounters and collects Performance Improvement data. - Serves as liaison for patients and their care team
- Ensure Compliance, adherence, and follow-up..
