Job Description

Under general supervision by the Cardiac Services Associate Director, with moderate latitude for independent initiative and judgment uses clinical knowledge to review medical charts for data abstraction and input in New York State Department of Health databases. Follows relevant regulatory policies, procedures, and standards of care for better outcomes and improved patient experience. Demonstrates effective communication, planning, and organizational skills and keeps current knowledge in the clinical documentation field and of all applicable regulatory regulations. The identified personnel perform related work and may perform tasks remotely, as authorized.

Summary of Duties:
  • Analyzes patient records for abstraction of data elements needed for NYS Department of Health cardiac databases
  • With chart review access, retrieves and prints clinical documentation in response to NYS Department of Health validation reports
  • Consults with medical and professional staff to review clinical documentation related to NYS Department of Health cardiac databases
  • Performs moderately difficult data entry and retrieval using appropriate programs and applications.
  • Meets weekly with Chief of Cardiology, Cath Lab Director or Associate Director to review NYS Department of Health extracted data points and fallouts.
  • Works closely with the Finance Department to extract information that is required by NYSDOH for completion and compliance of regulatory requirements.
  • Contributes to the strategic planning and process improvement initiatives and activities related to clinical documentation, by providing expert-level review and assessment, and effective recommendations or solutions for improvement.
  • Consults with medical, nursing and other staff involved in treatment to clarify issues and secure documentation as required.
  • Maintains appropriate files and other clerical records to insure timely review and processing of records.
  • Consults with personnel in other segments of the System involved in the financial, operational or professional components of health care program plans.
  • Participates in developing strategies, and alternative approaches for improvement of quality of care.
  • Prepares records for peer review orientation, Director of Cath Lab and Chief of Cardiology as needed.
  • Maintains appropriate files and other records to insure timely review and processing of records according to HIPPA compliances.
  • Prepares special Utilization Management reports, as required.
  • Assists with development of indicators for the evaluation of Quality Care standards within the service or the institution, as they relate to safety, adequate and appropriate health care.
  • Participates in staff meetings and may serve as a member of the Hospital Utilization/Quality Committee, as required.
  • Provides appropriate feedback to regulatory agency request for correction of data or clarification of data points submitted.
  • Fosters a good working relationship with the regulatory agency demonstrable through electronic communication and by attending workshops and meetings offered through the reporting regulatory agency
  • On a concurrent and/or retrospective basis identifies and reports trends and patterns of care which deviate from established norms that will have a negative outcome on the reporting periods.
  • Provides training and orientation to less experienced staff if needed and designated by Cardiology Leadership.

Database Management Responsibilities:
  • Maintains NYS Department of Health issued Access database
  • Attends required training for NYS Department of Health cardiac databases


Minimum Qualifications
1. Graduate of an accredited Medical Program, including Foreign Medical Graduate; or
2. New York State Professional Nursing Licensure (R.N.); and
3. Two (2) years of experience in a hospital clinical setting in a capacity which provides thorough
understanding of medical diagnosis, symptoms and treatment concepts; or one year of experience in
Utilization/Quality Management; or
4. A satisfactory equivalent.
