This mission of the Aging and Long-Term Services Department is to provide accessible, integrated services to older adults, adults with disabilities, and caregivers to assist them in maintaining their independence, dignity, autonomy, health, safety, and economic well-being, thereby empowering them to live on their own term in their own communities as productively as possible. Why does the job exist?
The purpose of this position is to provide a community-based alternative to institutional care. Monitor services for New MexiCare consumers in the program. Provide technical assistance, eligibility, and re-assessments in clients homes and communities. This position will assist in other projects and case management that pertain to vulnerable populations. Position will also provide tasks
of social service work involving planning, organizing, training and guidance when needed. This position performs highly responsible technical, professional and administrative work for the New MexiCare Home &Community Based Services. This position serves as the state New MexiCare Program Coordinator.
How does it get done?
Contractual and Budgetary - 50%
- Management oversight all aspects of the New MexiCare Programs in regard to program contractual compliance and performance measures.
- Program development and implementation of protocols and procedures for the New MexiCare Programs.
- Complete and review Single Needs Assessment (including service planning) for each participant and caregiver within New MexiCare Programs.
- Facilitate and provide guidance on service standards, policy and procedures for New MexiCare Programs
in the development and implementation of services provided to participants.
- Review contract budgets in regard to participant spending and costs. and ensure participant's spending is within budget and adjust if necessary.
- Complete spending plans, monthly reports and other required documentation timely.
- Submit budget and reimbursements authorizations monthly.
Case Management- 30%
- Complete in-home visits, phone check-ins and provide technical assistance to participants, caregivers, and authorized representatives.
- Ensure compliance with all program requirements.
- Assist participants and caregivers with resources, supports and procuring items necessary to keep them in their homes and communities.
- Collaborate with colleagues, managers, contractors, health professionals, and other state agencies to resolve complex and critical issues.
Outreach and Travel- 20%
- Conduct statewide outreach, training and education within the community on all New Mexicare Programs.
- Travel to participant homes and outreach locations.
Who are the customers?
The position works directly with participants, their Authorized Representatives, Power of Attorney, Caregivers, and ALTSD New MexiCare consumers in this program.
Ideal Candidate
The ideal candidate for this position would be someone who can multi-task, work autonomously, and provide outstanding customer service to a wide range of individuals. The person must demonstrate strong critical thinking, case management, working with diverse individuals, working in the field in clients' homes that may have health or safety concerns, conducting assessments, completing documentation, and ensuring all reporting is accurate and timely. This position requires travel and overnight stay.
Minimum Qualification
Bachelor's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care. Employment Requirements
Extensive travel throughout the state is required. Occasional overnight travel required, occasional after-hours and weekends required. Candidate will be required to successfully complete Defensive Driving Certification within 30 days of hire. Candidate will be required to successfully pass State and Federal Criminal background Check Investigation.
Working Conditions
Work is performed in an office setting and community setting with extensive use of personal computer, telephone, fax, copier, and other related office equipment. Occasional after hours and occasional weekends may be required. Extensive travel throughout the state.
